Iliotibial band syndrome (ITBS) is a common overuse injury affecting the outer part of the knee. The iliotibial band is a thick band of fibrous tissue that runs along the outside of the thigh, from the hip to the knee. ITBS occurs when the iliotibial band becomes irritated and inflamed, causing pain and discomfort in the knee area.

Causes of Iliotibial Band Syndrome

Common causes of ITBS include:

  1. Overuse: Repetitive activities like running or cycling can lead to ITBS due to excessive friction between the iliotibial band and the underlying structures, such as the lateral femoral condyle.
  2. Poor biomechanics: Factors like muscle imbalances, weak hips, or abnormal foot structure can contribute to ITBS.
  3. Training errors: Abrupt changes in training intensity, duration, or frequency can increase the risk of ITBS.

Symptoms of Iliotibial Band Syndrome

Signs and symptoms of ITBS may include:

  1. Pain and tenderness on the outer side of the knee, which may worsen during activities like running or descending stairs.
  2. Swelling or inflammation in the knee area.
  3. Stiffness or tightness along the iliotibial band.
  4. A snapping or popping sensation when the knee is flexed and extended.

Treatment for Iliotibial Band Syndrome

Treatment options for ITBS depend on the severity of the condition and may include:

  1. Conservative treatment: Rest, ice, compression, and elevation (RICE) to manage pain and swelling, along with pain relievers and anti-inflammatory medications.
  2. Physical therapy: Stretching, strengthening, and range of motion exercises targeting the iliotibial band, as well as addressing any underlying muscle imbalances or biomechanical issues.
  3. Gradual return to activity: As symptoms improve, a gradual return to normal activities and sports under the guidance of a healthcare professional.

Treatment Approaches

Ultrasound - Ultrasound warms deep structures, improves blood flow and flexibility and allows for better stretching.
TENS-TENS Unit - Unit is a low voltage electric current that stimulates nerves and produces a massaging or tingling sensation to decrease pain. Electrodes are placed directly on the skin on a painful point.
Superficial heat - A hot compress warms superficial structures, decreases muscle spasm and increases blood flow.Ice- Cold pack decreases redness and swelling and slows down metabolism during acute pain and inflammation.
Cupping - Cupping is used in physical therapy to increase blood circulation to a particular area, which can help to promote muscle healing, reduce pain, and increase range of motion.
Low Level Laser Therapy - Laser therapy increases blood flow, speeds up tissue repair and healing, decreases swelling and inflammation and enhances immune responses.
Joint Mobilization and Joint Traction - Joint mobilization and traction help to decrease pain by increasing mobility of a joint and decreasing swelling and inflammation.
Stretching - Regular stretching improves elasticity, increases blood flow, maintains healthy structure of joints and muscles and helps to prevent or slow down arthritic changes.
Exercise - Regular exercises increase muscle strength and joint stability, boost bone health and help maintain healthy weight.
Massage with Hot Stones - Massage with Hot Stones relaxes painful muscles and breaks down "knots"and restrictions. It has an overall soothing effect on the body, increases mood and improves wellbeing.
Bracing - A brace gives extra support to muscles and joints, especially after injury or surgery and when performing strenuous daily activities.
Patient Education - Patient is instructed on proper use of heat or ice, home exercises, proper sleeping and sitting positions and proper lifting techniques.

I have been coming for 1 month. When I 1st came I could not even walk a block without having to stop because my legs, back & feet were killing me. I have diabetic neuropathy in my feet going to my calves as well. I have been having tens unit & hot stone therapy on my feet & calves and it is FANTASTIC. I have also been doing the bicycle (started at 5 minutes and now I can do 15 minutes) and balance & stretching for my calves & thighs. Inna is a MIRACLE WORKER. Last week after I left her office, i went walking from Brighton 3rd St. to 1 block short of Coney Island Ave. Then I turned down the block to Brighton Beach Ave. I walked all the way down to Brighton 1st St. without any PAIN or TROUBLE WITH MY FEET. Inna Kouperman Of EXCEL PHYSICAL THERAPY is the ABSOLUTE BEST and I RECOMMEND HER HIGHLY.. I can't wait to see what the next months progress brings !!!!!

Inna is just a know it all physical therapist, from my knee to my shoulder injury, she was able to help me heal my injuries. She takes her time and every time there is a different type of excercise or massage applied to the injured area.
